To be eligible for workers' compensation benefits, you must be an employee of a company that has (or was supposed to have) workers’ comp insurance. You must have been injured at work or as a result of job-related duties. Treatment in Our Clinics On your first visit to our clinic, you will complete some forms regarding your injuries and we will collect some information about the company that you work for. Usually the company that you work for will provide you with a claim number. If you do not have a claim number, we will call the company in order to obtain approval of your treatment.
